You may have received a message saying that you are not eligible if:
- The birth date or Medicaid ID you entered does not match the information on file at the County Assistance Office.
- The Medicaid ID you entered is not correct.
- Your status is not currently active in the Community Health Choice (CHC) Program.
- You are not participating with Tempus as part of the Pennsylvania Community Health Choices (CHC) Participant Directed Program.
