Registry Search
The caregiver registry search function will be available beginning 10/30/2026. Prior to making the registry searchable by the public, the Agency is providing a 30-day notification period for caregivers who do not wish to have their information included in the public search, to opt-out of being included on this site. Caregivers who wish to opt-out of public listing may submit an opt-out request to the Agency during this 30-day period, along with any questions that they may have regarding this registry. Requests received within the applicable period will be processed in accordance with Agency procedures. After the 30-day notification period ends, the registry search will be available to the public, which will include the various provider types as listed in the bill. Caregivers who wish to opt-out after the initial 30-day notification period, may still submit their requests directly to the agency via the opt-out form.
This website provides general information about home or community-based services caregivers. Caregivers listed here appear because they meet the statutory criteria for inclusion in the registry under Oregon law. However, inclusion on the registry does not mean an endorsement or recommendation by the Oregon Department of Human Services (ODHS), nor does it guarantee a caregiver's quality or performance. Information displayed on this site is based on data provided to ODHS and may change over time. While ODHS maintains the registry as required by law, ODHS does not independently evaluate or verify a caregiver's skills, training, or professional competencies beyond the statutory requirements for certification and background checks. Additionally, not all providers may be included/listed on this Registry, as some providers may choose not to be included on the Registry, or due to the timing of when data was submitted to ODHS. Users are responsible for conducting their own review and verification of any information relevant to their hiring decisions. ODHS is not responsible for decisions made or actions taken by users based on information obtained through this site. Users assume all risks associated with the use of this information.
