The Pennsylvania Community HealthChoices (CHC) plan, through Direct Care Careers, supports this platform to identify Direct Care Workers for the Participant-Directed program. The platform matches the Participant as the employer to a Direct Care Worker whose skills will allow them to maintain their independence in the community.
FOR PARTICIPANTS IN THE COMMUNITY HEALTHCHOICES (CHC) PROGRAM:
- Current Participants in the Pennsylvania Community Health Choices (CHC) Participant Directed Program are eligible to create an account.
- You need your Recipient ID to create an account.
- This is your 10-digit Medicaid/State ID (MAID) on your insurance card (this is different from your Insurance Participant ID number).
- If you do not know your MAID, you can contact your insurance provider’s Member Services Department.
- If your Recipient ID is active your account will be created.
- If you are not an eligible CHC participant, please contact your County Assistance Office.
FOR COMMON LAW EMPLOYERS (CLEs) IN THE COMMUNITY HEALTHCHOICES (CHC) PROGRAM:
- You need your 6-digit Employer ID (R Number) to create an account.
- Your 6-digit R number begins with the letter 'R' and can be found on the Tempus Participant Dashboard or the Employee Self Service (ESS) portal.
- You can also contact Tempus at PAFMS@TempusUnlimited.org to obtain your R Number.
FOR DIRECT CARE WORKERS (DCWs) IN THE COMMUNITY HEALTHCHOICES (CHC) PROGRAM:
- DCWs with a good-to-go (active) status in the Pennsylvania Community Health Choices (CHC) Participant Directed Program are eligible to create an account.
- You need your 6-digit Employee ID (E Number) to create a DCC account.
- Your 6-digit E number begins with the letter 'E' and can be found on your Tempus pay stubs or the Employee Self Service (ESS) portal.
- You can also contact Tempus at PAFMS@TempusUnlimited.org to obtain your E number.

Direct Care Careers matches Participants and Direct Care Workers based on things that are important to the Participant, like location, availability, and skills. This makes it easier for both Participants and Direct Care Workers to connect quickly and get services started.
